A more complete brochure is attached about the VHA Care Coordination / Telehealth Meeting April 4-6. This is particularly targeted toward those of us with interest in developing a telehealth effort (especially focusing on home health for the frail elder) and wanting to coordinate with our local VISN / VA site.

My apologies for another email about this mtg but I am attaching the latest/final brochure/agenda for the mtg. Please note that there will be a session on Wed afternoon on April 6, specific to IHS/VHA collaborations. I have just spoken with Adam Darkins from the VHA, who is very hopeful that this meeting will be productive for specific collaborations between I/T/U and the VHA.

Attached please find the registration form for the VHA Care Coordination and Telehealth Leadership Forum scheduled for Salt Lake City April 5-7. I learned that we cannot register on-line (as the on-line registration form is behind the VHA firewall). Folks on this email have either indicated they are attending the meeting or expressed possible interest in attending. I have also copied all the Chief Medical Officers (although some are already attending the meeting) so that you have the form in case others in your Areas are interested in attending the meeting.
